Serious Incident Project Board

The SIPB continues to meet at Alder Hey on a monthly basis. PITY II's representatives are currently Jan Robinson and Sheila Rigby.

At the last meeting on 15th Jan it was reported that the trust has had 4,095 total inquiries so far. There were still 350 cases where families were waiting for the release of organs. It was stated that it is currently taking about 4 weeks for the trust to get back to families with an answer as to whether they were involved in organ retention or not.

The hospital is contributing to the coroners review process, and as a result of the Follett Review into the role of universities; meetings have taken place between the Chair of the Trust and the Dean of the Medical School.

The Trust now has a bereavement co-ordinator in post, Brian Langrick, who used to manage the Alder Centre. This service aims to improve the way that the hospital deals with death and issues around it. Sue McQueen, who used to be one of the link workers, is also helping Brian on a part time basis.

The Trust is also considering setting up some special independent support - an independent project manager will be appointed to organise this, and a presentation about it will be made to the next meeting.

The Trust has achieved all the actions needed from the Underwood report and will be applying for national accreditaion for its pathology services. The new mortuary is under construction.
