Phish:

We are in the process of formulating a policy which specificlaly addresses
MP3 and other online distribution of Phish music. In the meantime, I would
suggest that studio material, however rare, will always fall outside the
band's policy allowing taping and trading regardless of the media or
format. We realize many people are looking for guidance on these issues so
we'll move as quickly as possible in finishing and publicizing the policy.

All other material has been used without express consent, mainly because it has become impossible to get a straight response from people about this. So, if something on here is yours (and you can provide copyright/patent/trademark information), then I will happily give you credit or take it down. Your choice.
