This is the Registration Form of the website of UPLB People at http://uplb.go.to . You can print this Form from the website, it can be photocopied, and it can also be secured from any UPLB Kaiban member. Instruction: Supply the information required below. Be diligent and serious in accomplishing this Form because what you write here will be published in the Internet and changing the entries will cost you. Your registration may be nullified on any of the three grounds: (1) you are/were not actually connected with UPLB, (2) your information entry is not truthful, or (3) you did not follow instruction on payment and submission procedure. Please attach a photocopy of your UPLB ID or UPLB Form 5. We don't refund payment of nullified registration. In accomplishing this Form, please use typewriter or write legibly to prevent the encoder from committing typographic error.

Okay. First, paste your best/favorite picture over the photo sketch. For best result, use a colored photo professionally shot in a studio. We ask for a pocket-size or junior-size picture, but if you don't have one, any size will do.

Payment

As of March 2001, the homepage construction price we charge for a Student, Alumni, or Faculty/Staff is only 300 Pesos (one-time fee) and the price of changing entries in a pre-existing homepage is 100 Pesos (visit the website for price update). The only method of payment is by making an over-the-counter deposit to the account of John O.R. Petalcorin in any of the following banks:

IMPORTANT: After rendering a bank deposit, attach the original bank deposit receipt (get 2 duplicate copies if you want to keep a copy) at the back of this Registration Form and place it in a long letter-envelope for submission. DO NOT put any money in the envelope. Address the envelope to John R. Petalcorin, UPLB Website PO Box, UPLB Post Office, 4031 College Laguna.

Submission

Option 1. Drop it in the uplb.go.to drop box at Ankers Restaurant, Grove, College UPLB.

Option 2. Submit it to any member of UPLB Kaiban, UP Los Baños Campus (if you know one).

Option 3. Mail it directly to the UPLB Post Office (requires a Stamp).

What You'll Get

We will build you a personal homepage and link it to the index of your Freshman Batch, Graduation Class, College/Course, and Organization/s at http://uplb.go.to. Later, if you want us to change any entry in your homepage just read the Services/Rates section in the website. The acknowledgement receipt will be e-mailed to you or be mailed to the mailing address you indicated in your registration form. Your URL (Universal Resource Locator) will be indicated in the receipt. Thank you for signing up.
