ADOPTEES WHOSE ADOPTIONS WERE
FINALIZED BEFORE JANUARY 1, 1964
Adoptees whose adoptions were finalized prior to January 1, 1964 have access to their original birth certificates. They can obtain these by requesting them from the Bureau of Vital Statistics. The process requires filling out an Affidavit of Adopted Person and having it notarized, paying the fee and presenting two pieces of identification. A drivers' license and social security card are acceptable. If you need to apply for this information by mai they will allow you to make copies of your id so it's more convenient.

which is a mutual consent registry. For an adoptee to register, he or she must be 21 years or older and have filed a petition with the county probate court ,
which costs up to $50 (in Cleveland it's $25). This will release your birth information into the registry to be matched with that of your birthparents, if they have registered. Of
course, for bparents the registry is free.
