Please use whichever of the forms provided best suits you. The old form is still available for those uncomfortable with change, or with computers that are not up-to-date. You may still print out the form's two pages, fill in your list totals, and mail it, with the $6, to Canadian Listers' Corner, 124 Clarendon Avenue, Ottawa, ON K1Y 0R3, the address on page two.
However, new forms have been developed to facilitate the data transfer to the compiler. These are interactive forms which you can fill out online, then by pressing the submit button, you can create a file to send the data to the compiler via email. There is an option for printing a copy for your records. There will likely be problems related to various email systems and operating systems, so you may wish to print a second copy and mail that to me, if it proves easier.
These forms have been designed to accommodate the lists in a logical sequence which should eliminate the usual placement errors. Nothing, I'm told, ever goes according to plan, so I expect some of you to have problems. If so, please send any questions by email and I'll do my best to help. And for now you will still have to send the $6 by mail.
As a Statistics Canada employee, I can almost hear the groans from the folks out there when I used the word "forms", rather than "form". But don't worry; you need only fill out one. I have simply tried to make it easier for those who only submit numbers for major categories. Because the complete form is 12 pages long, I have provided a 2 page Major Category Form that may be used by those not submitting local lists.
This process will be evolving over time, hopefully to a much better system as I learn more; perhaps to a completely automated system at some stage.
