| Date : 1 July 2003 Dear Parent / Guardian We are eager in promoting positive youth development among our students. To assist us, we are conducting a survey of our students in relation to various areas of their development (i.e. their thoughts, feelings and behaviors in relation to school, family, friends, and themselves). This anonymous survey in July/August 2003 is arranged with the assistance of two Malaysian PhD students in Human Development & Family Studies at The Pennsylvania State University. We are inviting students to voluntarily participate in this study, which is one class period long. They will be able to skip any questions and stop at any time. Through this survey, some students may become more aware of how they think/feel/behave in relation to their positive development and there are no significant risks involved. We welcome your feedback or interest in this survey. You are free to decline your child from participating by returning the consent form below to the school office by July 7th 2003. Thank you.
