Assign
a category/title and page number to each section of your portfolio. |
3. Autobiography |
Write the story
of your life. Include information about when and where you were born.
Describe your immediate family (pets included); the schools you have
attended; your favorite courses; your hobbies, outside interests,
and special talents you have or awards you have received. Speak of
your plans for the future. Don't forget to include a current picture
of yourself! (Photocopy your ID card if you don't have a current picture) |

4. Physical Fitness
Data |
Attach to the
front of this page your Physical Fitness Data cards for both 1st and
2nd semesters of your freshman year at Lowell; to the back of the
page, attach the Physical Fitness Data cards for both the 1st and
2nd semesters of your sophomore year. Label each card with the appropriate
Semester and Class. You may enclose this sheet in a plastic page protector. |

5. "Moving For
Life" Text Notes |
You may divide
this into three parts to accommondate vocabulary words, the outline
and the study questions. |

Other Requirements: |
a. Type everything. |
b. Use dividers
(colored, if possible) and plastic paper protectors for any sheets
you do not want to redo. |
c. Proofread
your material to eliminate spelling and grammatical errors. Errors
such as these definitely detract from the effectiveness of your written
presentation. |
d. Contain the
portfolio in a 3-prong binder which has a flexible plastic cover.
Remember, you will be collecting Physical Education materials for
two years. |
