REGISTRATION CHECKLIST

 

TEAM MANAGERS: If you can check off every item on this list, your registration will be quick and simple.

 

TEAM REGISTRATION:

 

__ Team Affiliation/Registration Form for 2008 (“Roster”) (original and 2 copies)

__ Minimum of 11 players, maximum of 25

__ Copy of Certificate of Insurance (field permit)

__ Check or proof of payment for team registration fee of $100

__ Check or proof of payment for new or outstanding performance bond monies

__ ALL player registration requirements listed below

 

PLAYER REGISTRATION REQUIREMENTS:

 

Returning Player:	New Player:
·        Amateur Player Registration Form (A-Form)* ·        Prior GLWSL Player Pass Card	·        Amateur Player Registration Form (A-Form)* ·        Copy of State ID or Driver’s License ·        Photo (1” x 1”, Color or B&W, Headshot) ·        New GLWSL Player Pass Card**

 

*A-Forms must be:

·        Fully completed

·        Signed by the player and team manager.

 

**New pass cards must have:

• Player signature;
• Photo: color or B&W photo (no photo paper) and attached by glue only
• ID number completed (league # = 004, your team # , pass card #) and;
• Back of the card completed, including Club (team name), League (GLWSL) & Season (2008)
