Mission:

"Helping patients and their providers to effectively detect, treat, and manage diabetes in Fairfield County."

Goal:

Our major goal is to provide a center for diabetes education which consists of a miltidisciplinary team whose primary goal is to provide detailed education for referre patients regarding their disease, recommended diet, monitoring, and medications. Members of this education team include Certified Diabetic Educators, Pharmacists, Exercise Physiologists, Registered Dietitians, and Health Educators. We provide a variety of services both in the Diabetes Education Center as well as in the community.

-Exercise Education & Recommendations
-Blood Glucose Monitoring
-Oral Medications
-Insulin Administration
-Stress Reduction/Goal Setting/Community Resources
-Foot Care
-Exercise Recommendations

Community screening and education

Educational services (seminars, educational supplies, etc.)

Database management to track diabetes in Fairfield County

Information center/individual assistance with indigent care programs

Management of applications for diabetes related community grants for Fairfield County

Serve as an educational site for students of healthcare professionals


Funding:

The Fairfield Diabetes Initiative Group is a non-profit organization founded in 1999 by local health care providers and other interested community leaders. This organization was started in an effort to assist in the detection, treatment and education of Diabetes in Fairfield County, South County. Under the guidance of the statewide Diabetic Initiative of South Carolina, our organization operates as a committee of the Fairfield Business Education Partnership, a non-profit organization dedicated to providing educational services in many areas of the community.

Funding for our organization is from a variety of sources including the Duke Endowment Program, the United Way, pharmaceutical companies and organizations, and local businesses and individuals.

The American Diabetes Association Recognizes this education service as meeting the National Standards for Diabetes Self-Management Education.
