The Horadric Cube is a quest item you must have to complete the main quests in Act Two and Act Three.  However it's much more than just a quest item, it has a ton of other cool features that make it useful throughout the entire game.

All these recipes work in Diablo II and the Expansion.

6 Perfect Skulls + Rare Ring Rare Ring
3 Perfect Gems + Ring Magical ring. Apparently disabled in v1.09. Any 3 types of perfect gems can be used.  New magical rings/amulets have random modifiers, potentially very good. This formula works with ANY magical item. (Ilvl = 100.)*
3 Perfect Gems + Amulet Magical amulet.
3 Amulets 1 Ring Formula for quality:
  Max Item level = [.75*Clvl]+3

3 Health potions + 3 Mana + 1 Gem 1 Full Rejuv Potion You may use any quality of potions and gem for this one.
1 health potion + 1 Strangling Gas potion 1 Antidote Potion You may use any quality of red potion.
Recipes below this are enabled in v1.09 on and off the Realms.  ***
6 Perfect Gems (one of each type except skull) + Amulet Prismatic (16-25% Resist All Prefix) Amulet Nice way to get some resistance on an amulet, if you can't find one any other way. May occur with a random suffix as well.
1 magical ring + 1 perfect emerald + 1 Antidote Potion 1 Jade Ring These all give a ring with 21-30% resistance to the particular element, and can have a suffix as well.  The v1.08 big where the mods would add onto set/unique items at times is apparently fixed.

All now require a perfect gem, and the Lightning ring has changed ingredients, it used to be 1 ring + 2 topaz (any quality).

1 magical ring + 1 perfect ruby + 1 exploding potion 1 Garnet Ring
1 magical ring + 1 perfect sapphire + 1 thawing potion 1 Cobalt Ring
1 magical ring + 1 Perfect topaz + 1 rejuvenation potion 1 Coral Ring

* Ilvl = #.  Various recipes are Ilvl 40 or 60 or 100. This means that the affixes you will generate on the Rare item will have a maximum Qlvl of this number.  King's is Qlvl 56 in v1.08, for instance, so it's possible to get on a recipe where the Rare can have Ilvl 60 or 100, but not 40.  See our Modifiers section for full Affixes info.
**
Three chipped >> one flawed.  Three flawed >> one normal.  Three normal >> one flawless.  Three flawless >> one perfect.  Three perfect don't Transmute to anything.
*** These recipes were all buggy in v1.08, they were adding the bonus stats onto random magical items, and even Rare, Set, and Unique.  For instance you could get a set or unique ring with bonus resistance, or a Unique amulet with prismatic.

** "Non-normal" item means anything with magic on it: Magical (blue), Rare (yellow), Set (green) or Unique (gold).
***
"of spikes" = Suffix, target takes 2-6 damage.  Clvl 15 requirement.

Quest Transmutations

The cube is needed to complete some quests in the game.

Ingredients: Results: Notes:
Staff of Kings
+ Viper Amulet
Horadric Staff Needed to open the way into Tal Rasha's Tomb in Act Two.
Khalim's Eye
+ Khalim's Heart
+ Khalim's Brain
+ Khalim's Flail
Khalim's Will Needed to smash the Compelling Orb and enter the Durance of Hate in Act Three.

All of these items have decent stats on their own, and Khalim's Will is the fastest swing item in the entire game and has no Clvl requirements to use it, making it a popular item to twink for a new char.

Much to the delight of fans, the Cow Level lives!  Access to it can be gained through using the Horadric Cube. You must have 1) beaten Diablo on that difficulty level, 2) The correct ingredients, 3) You only get to open the portal once per difficulty level (though you can go through someone else's portal if they open it.)

Ingredients: Results: Notes:
Wirt's Leg
One Tome of Town Portal
A Red Portal will appear to The Cow Level. The Cow level used to gain you alot of experience but now v1.10 made cow runs not worth doing.
Remember if you kill the king you will no longer be able to make a cow game again
