The Personal Support Plans (PSP) available here are written from the client’s perspective to emphasize a person-centered approach. They provide an exercise in putting ourselves in their shoes and seeing the world from their perspective. 

By writing the plan in a first-person format, use of psychological jargon is minimized and a simpler and more user-friendly style is encouraged.   (I know it has helped to teach me to write in a more understandable way!)

The PSP melds more traditional concepts about active treatment  and the newer person-centered ideas about supports.  The supports in the PSP follow directly from a functional analysis of the person’s behavior and analysis of personality.  Statements about the person provide the outline for spelling out recommended supports. 

Four overlapping sections are included to ensure attention to critical  components of a comprehensive plan:     1) Respect me as a person ...       2) Set me up for success ...
3) Teach me better ways ...      and      4) Help me through tough times .... 

The PSP format is set up to allow the PSP to evolve.  The PSP should be updated as staff have new insights and understanding of the person’s behavior. 

The PSP format includes data collection on selected behaviors and symptoms reflecting on the effectiveness of the included behavioral supports and also any pharmacological interventions.  Additional supplemental data collection can be developed for particular procedures/supports (e.g., see sample about desensitization). 

The PSP format includes forms for obtaining necessary approval and training in the plan.  Approvals are required from the team, any family/advocate/guardian, as well as the person’s physician and any psychiatrist.  All staff need to be trained in the specifics of the plan. 

The PSP format is set up to allow ongoing review of progress.  PSP Reviews provide a format for completing regular review of data, updating the analysis of behavior and personality, and providing specific recommendations for team consideration.
