1. Personal Support Plan (PSP) (04/01/01, edited 07/14/01) -- a sample of a complete plan, also includes a medication reduction plan (especially applicable in ICFs). 2. PSP Merge Document (updated 07/22/02) (copyright 2002) -- includes the merge file for creating a PSP from scratch; numerous options are included (e.g., 28 day vs. monthly data collection, med reduction plan, graphing of data, etc.) so some editing/deleting will be required; includes introductory instructions and comment inserts throughout document. Note: Before trying to activate the PSP Merge Document you will want to download, unzip, and save the data document. 3. Desensitization (04/02/01) -- an extract from a PSP in this case about helping the person deal with going to the Podiatrist, includes a data form. 4. One:to:One Supervision (04/13/01, edited 07/14/01) -- an extract from a PSP about providing more intense supervision, includes additional documentation forms including Sleep Charts and Safety Techniques. 5. Discussion Notes (04/22/01) -- an extract from a PSP about encouraging a "sit and talk" strategy for resolving conflicts. 6. Self-Monitoring (04/23/01) -- an extract from a PSP designed to increase the person's involvement in monitoring his own behavior and hopefully internalizing some controls, includes some unique forms. 7. Sample Requests for PSPs (updated 10/02/01) -- lists of sample requests and supports that can be included in PSP; organized by topic (27 sections) and then all repeated again organized by client (28th section). 8. Peer Cooperation (added 04/01/02) -- this is a sample of a complete PSP for a fellow living in a small ICF-MR; includes supports related to it "not being easy living with 7 other guys" (substitute your own #!); also includes med reduction plan, self-monitoring, and more detailed Behavior/Intervention Checklist. 9. Outings Eligibility (added 04/04/02) -- this sample includes a "Community Outing List" for listing outings and defining any behavior eligibility requirements depending on the associated risk of the activity (although it may not be acceptable to punish behavior by canceling an outing, it might be advisable to re-consider "readiness" for an outing given recent difficulties with behavioral control). 10. Graphing Data (added 02/15/03) -- includes format for creating graph using Word and its graphing program and/or transferring info to Excel for maintaining multiple client graphs using one common data sheet. Usable with or without PSP formats. 11. Social Stories (added 05/18/03) -- includes use of mood checks, discussion notes, and social stories to supports an individual with PDD, limited verbal communication, but some academic skills. 12. ... to be added (check back another day!!!) |
