DR Mangano is involved in all outcomes of research addressing perioperative cardiac morbidity, mortality, and stroke. Over the past several decades, Dr. Mangano has focused on the problems of perioperative myocardial infarction, investigating the predictors of infarction in high risk patients undergoing non-cardiac surgery . Based on this reearch therapeutic trials have been conducted investigating two principle areas of research: the use of adenosine thrapies in high risk patients undergoing non-cardiac surgery, and the use of adenosine compounds for prevention of reperfusion injury in patients undergoing cardiac surgery.
In addition to the detection and prevention of myocardial infarction, Dr, Mangano and his associates are investigating the etiologies for perioperative stroke and cognitive dysfunction, particularly in patients undergoing cardiac surgery. To this end, several large scale trials have been completed, identifying  predictors of stroke, and other peroperative risk factors for stroke. In this setting, several therapies have been assessed, including anesthetics as well as adenosine-related compounds. These studies, addressing perioperative stroke and cognitive dysfunction, are similar to those addressing perioperative myocardial infarction in that they are conducted in multiple centers, requiring numbers of patients in order to investigate effects on adverse outcome

Dr. Mangano is also Director of the Multicenter Study of Perioperative Ischemia (McSPI) Research Group, consisting of investigators in approximately 150 centers in the United States, Canada and Europe. The primary goals of this search group are to investigate critical problems in high risk patients, as well as to develop for approximately five years and well, developing junior investigators and fellows.
