Students interested in OJT need to have permission from Mrs Hofstrand and Ms Beasley.

Once accepted in to the program, students need to have:

	up-to-date class schedule
	a parent permission signed
	training agreement processed, the original is to be kept at the work site.
	completed student training plan

Your personal folder will be kept in Mrs. Beasley's room. You need to make sure that the forms are kept up-to-date. Your report card grade will reflect how current your folder is.

Time Cards-signed by your supervisor
Check stubs - xerox or originals - supporting time cards
By Semester
Report card signed by supervisor
